What is GERD?

Gastroesophageal reflux disease (GERD) occurs when stomach acid often flows back into the tube that connects your mouth and stomach (esophagus). This backwash (acid reflux) can irritate the lining of your esophagus.

Many people experience acid reflux from time to time.GERD is a mild acid reflux that occurs at least twice a week, or a moderate to severe acid reflux that occurs at least once a week.

Why do patients develop GERD?

The cause of GERD is unknown, although it could be due to an abnormal relaxation of the lower esophageal sphincter (LES), a muscular valve in your esophagus that keeps food from flowing back up your throat and stomach acid in your stomach. Increased frequency of transitory sphincter relaxation, increased pressure from the stomach due to a hiatal hernia, and increased pressure within the abdomen are all possible causes. Stomach acid can reflux into the esophagus if the LES opens too often or does not seal tightly enough, causing the burning sensation.Other causes of GERD include gastric emptying disorder or failed esophageal peristalsis (wave-like movements in the muscles).

How can you know if you have severe GERD?

Before surgery can be considered for people with severe GERD, your doctor must offer objective documentation of gastroesophageal reflux. The flexible esophagoscopy is frequently used as the first step in determining the diagnosis. If there is evidence of esophagitis, tissue biopsies may be taken during the endoscopy procedure (inflammation). Barrett’s esophagus diagnosed histologically from a tissue biopsy is also regarded objective evidence of GERD. The 24-hour esophageal pH test, which must be inserted by your physician in the absence of endoscopic evidence of reflux, is the current gold-standard objective test.

Frequently Asked Questions About Anti-Reflux Surgery

You should be able to go home the next day. You should be able to return to work after 3 to 4 weeks, depending on how much surgery you need and your type of work. Regular exercise should help you to return to normal activities as soon as possible. Long-standing GERD can lead to a precancerous condition called Barrett’s esophagus, in which cellular changes have taken place in the lower esophageal lining. This condition can progress further into low- or high-grade dysplasia, and then to esophageal cancer. For this reason, patients diagnosed with Barrett’s esophagus are monitored regularly with endoscopic surveillance and biopsy.

In most cases you will always take medications to control symptoms of GERD, unless you have had surgery to tighten the lower esophageal sphincter. This surgical procedure prevents stomach acid from entering the lower esophagus.

Approximately 95% of patients have great surgical results, and more than half of them are able to live without taking acid reflux drugs. Symptom improvement has been found in studies to last for a long time. Even though the success rate is excellent, 10 to 30% of patients will experience reflux symptoms again. However, some people may require a repeat operation in 8 to 10 years. Bloating, trouble swallowing, and diarrhea are some of the other postoperative problems.
